Use this form to request the transfer of the WHOLE balance of your superannuation benefits between funds. Caution: when you transfer your superannuation, your entitlements under that fund may cease. You need to consider all relevant information before you make a decision to transfer your superannuation. If you ask for information, your superannuation provider must give it to you. Standard Choice Form Publications - 19 February 2010 The Standard Choice Form sets out employee options for choosing a superannuation fund. Your employer is obliged to provide you with a Standard Choice Form within 28 days of commencing employment or you requesting the form. This form allows you to give your employer the details of the fund you wish to have your contributions paid into.
